#179 Add bounded knowledge retrieval

Adds bounded, lossless knowledge retrieval for BCQuality review skills. Catalog pages are capped at 16,000 UTF-8 bytes and return every applicable row with no ranking. Article and sample bodies are fetched exactly in chunks of up to eight paths. Each review leaf gets private GUID-scoped scratch and report paths, and its return is validated deterministically before rollup.

  • Each review leaf gets GUID-scoped scratch and report paths, and its Task return is validated deterministically before rollup
  • The bounded Source workflow now applies to all 16 Microsoft review skills; their Relevance, Worklist, Action and Output rules are unchanged
  • Behaviour changes for users or extensions.
