#202 Restore deterministic review fixture coverage

Quality review fixtures now register 23 knowledge articles with deterministic coverage, including retrieval keywords to ensure clean and positive controls remain discoverable within the bounded candidate set.

  • Preserves all existing fixture selections while adding three narrow retrieval keywords for better test control selection
  • Keeps coverage waivers empty and validates through comprehensive synthetic and final-diff gates with zero waivers
  • Adds routing metadata: credit-memo-routing for al-comments article, rec-modify pattern for pages article, subtype-test for testing article
  • All 19 existing review leaves and skill indexes preserved; passes knowledge index, skill schema and review contract validation
